At least one is required — add your Basic Training as a minimum. Leave the expiry blank only if the certificate genuinely never expires.
Only if you hold a licence or qualification above the rank you are applying for — for example you are applying as Able Seaman but already hold your Third Mate licence, or you have passed a board examination or taken a higher degree. Tell us the rank it licenses you for; that is what tells us you are ready for promotion. Leave this empty if it does not apply to you.
List the vessels you have served on. Durations are calculated for you.
